YOU WILL NEED THESE ITEMS IN FRONT OF YOU:

One of any of these five forms of Identification:

- Your driver’s license

- Your Social Security number

- Your I-94 Card

- Your Green Card, Visa, or Permanent Residency Card

                 - Your Country or State Issued ID Card

                AND

                   Your child’s school Identification number

                              Your child’s Date of Birth

If you are not able to provide the above information you would NOT be able to meet the criteria for assistance from Star-Telegram Charities Inc. / the Goodfellow Fund.
